I been shooting Winchester NATO 124gr rounds. I have 3-26 round mags, 1-17 and 1-20. It seems to happen on all of them at various times. I have small hands (unfortunately) and I shoot left handed, so I doubt that I'm riding the slide lock. Been shooting for 10 years now, so I doubt I'm limp...

XDMan, Thank you so much for your info. As you can see from my picture I was able to remove the screw from the sight just as you instructed. Now my questions are how do I zero the sight and how much torque do I use to keep it in place, if torque is the right word. Hard to believe that this set...

In the pictures you can see two views. The top and bottom. The top shows the missing piece of the screw. The bottom one shows the remainder of the screw. Do you have the A14B plate? If so, can you send me a picture of the sight from the top view? Thanks

Any recommendations on a specific Torx driver?

The screws that hold the plate to the slide has red loctite. I just need a new sight

The plate is red loctited as the blue wouldnt hold the plate to the slide for very long, about 100 shots. So I used the red and I do have a torque driver and torqued to the the appropriate specs. The optic is blue loctited to the plate and can be remove easily.

Waiting to hear back from Springfield on getting a new sight that will fit the A14B plate. They want me to send back the plate but it is red loctited to the slide. Love to video!

I wish I could loosen that screw. There isn't anything to grab to screw on the bottom.

Upon further examination it seems that the screw had broken off the sight and some of the screw remains in the bottom of the sight, flush with the bottom making it impossible to grab with a pair of pliers. (see pics). Also, there isn't a screw hole on the A14B plate for the sight screw to screw...

I do have a Torx set and a torque driver. Thanks for the tip!

I did contact SA and they will send me the set screw. I wanted the size of the screw to make sure I have the appropriate screw driver
