OMG-I have no words. What a nightmare

Lock out/tag out. OSHA will be all over this. The kid probably never had any of the proper industrial training.
And even if he did, what are the odds he was following what was taught. Having spent a significant amount of time as a Senior NCO during my military life, I can’t count the number of disciplinary actions initiated for folks not following prescribed “technical orders” which were required to be followed step by step to prevent mishaps. Inevitably the person was found out to be NOT following them because something went very, very wrong, resulting in injuries or millions of dollars in damage. Human nature, whether 1985 or 2025…some folks will always take shortcuts.
 
Umg, what a horror. The people who were there will be haunted for life. When I was 15 I got a job working at a rock crushing quarry. Sometimes the machine would get jammed up. There was a series of steps that were to be followed but the guys made an adhoc faster solution of standing on top with a steel pipe and poking stuff loose.

Naturally , one afternoon my friend who worked there, a couple, years older, slipped and fell in, his legs were crushed to he hips. He was in there a long time screaming, passing out and screaming again. I still have nightmares of his face and screams.
 
There is an aluminum plant local to me and a few years back a catwalk failed and 2 guys fell into a molten pot of aluminum. One guy was killed outright and the other grabbed part of the broken catwalk and only fell in to his waist. He was pulled out while begging to be let go all the way in. He lived less than 24 hours.

Manufacturing accidents are horrid!
