Yes, I have seen this happen twice with my wife's pistol. I suspect ammo as well. I have had a jam in an m16 in the field that behaved the same way. One option is trade it off for a .357 Model 19 revolver.
As far as cleaning....I shot on a pistol team in the USMC that was led by a distinguished master. Jim could shoot 10 rounds down range and grouped so tight rounds were going into the single hole. I could not see 10 distinct .45 holes. He rarely field stripped and cleaned his pistol. He would lube the top of the barrel with the slide locked back, let the slide go home.. lube the barrel at the ejection port.. work the slide a few times and then take 1st place in match after match. His gun would go in the case and then come out for the next match. Get a lube job and shoot rounds on top of one another. Do we spend too much time field stripping and cleaning weapons? Jim also would use a brush on the face of the bolt to clean it. That was about it.
