IDK guys if all those things happened I might wanna take up another hobby. Ive fired more than my share of open bolt SMGs STEn Sterlings etc and never had an issue
Im gonna add that I did have a crossbow fail once while hunting a vampire; but lucky for me I was able to use my back up wooden stake 🤦‍♂️...always carry a back up

How did I know it was a vampire? It hid under the bed and turned to dust. When I looked I found the dust
 
Open bolt smg's. In older guns where recoil springs have gotten weaker, magazine feed lips have probably spread and ejectors, extractors have worn down or weakened and no proper maintenance has been done (probably because of lack of parts) . An out of battery detonation can occur. Key note older guns , lack of proper maintenance.
I have worked on several WW 2 weapons in the arsenal of some departments ( weapons purchased thru the DOD) these are common issues I have encountered doing normal maintenance. So yes it's possible for Belt Fed to have incurred an O.O.B.D..
MG 34's and 42's great high rate of fire weapons are very well built. However correct installation of parts and reassemble of parts is crucial. Yes I have disassembled cleaned and reassembled a mg 34. It had a broken seat and disconnector, it would run away and not stop firing until out of ammo or you twisted the belt till it jammed.
