accidents do happen..LEO's are under a lot of stress, and to maybe possibly holster or remove the gun in the actions of a tense situation, well......accidents......do happen.

Holstering or unholstering, still need to keep finger off the trigger, poor or little training or incompetence.

Several companies products leave large gaps between trigger and holster.
