“Cleaning” a loaded firearm-what could possibly go wrong?🙄. My old Dad told me nearly 70 years ago “son, be very careful around “unloaded” guns, those are the ones that accidentally kill people”. A wise man (and the reason I don’t “play” with guns today.

Hmm, my dad taught me live ammo and gun cleaning always occupy different rooms. And after I checked a gun is unloaded and then double checked to be certain I should check one more time.
Lenny the owner of "The Glock Store", always does his public service announcement of "no ammo in this room", when he tears down a Glock to replace any part his company offers to install in a Glock

i follow that rule too in cleaning ANY gun that is getting worked on.

seems to be a simple rule to follow, yet some for whatever reason, simply do not, whether it be from absent mindless, or ignorance.

A guy that stupid was probably not cleaning his gun. He was probably posing for selfies of him being a "tough guy" for his Instagram account and his dog, the brains of the operation obviously, had had enough of that nonsense.
